Notice of Privacy Practices
Who we are
RevCycleMGMT provides revenue-cycle management, billing, and practice-management services to healthcare practices. When we handle your health information, we do it on behalf of your healthcare provider and under a written agreement that requires us to protect it.
How we may use and disclose your health information
- For treatment. Sharing information with your doctors and caregivers so they can care for you.
- For payment. Our main job: billing your health plan, checking coverage, processing claims, posting payments, and sending statements.
- For health care operations. Quality checks, audits, training, and compliance reviews that keep services safe and accurate.
- As permitted or required by law. Public-health activities; reporting suspected abuse or neglect; health oversight; court orders and lawful requests; preventing a serious threat to health or safety; workers’ compensation.
Uses that need your written authorization: most uses of psychotherapy notes, marketing, and any sale of your information. We never sell health information. You may revoke an authorization at any time, in writing, except where we have already acted on it.
Your rights
- See and get a copy of your health information, including an electronic copy.
- Ask us to correct information you believe is wrong or incomplete. If we say no, we tell you why in writing and you may add a statement of disagreement.
- Get a list of disclosures we made, up to six years back, other than treatment, payment, and operations.
- Ask us to limit what we use or share. If you pay for a service in full, out of pocket, and ask us not to tell your health plan, we must honor it (narrow legal exceptions apply).
- Ask for confidential communications — for example, mail to a P.O. Box, or no voicemail messages. We accommodate reasonable requests.
- Get a paper copy of this notice at any time.
- Choose someone to act for you (medical power of attorney or legal guardian).
Signing an acknowledgment of this notice is requested, but never required to receive services.
Our duties
We are required by law to keep your health information private and secure, give you this notice, follow the notice currently in effect, and tell you promptly if a breach may have compromised your information. If we make material changes, we post the updated notice on this page and make copies available.
Complaints — no retaliation, ever
If you believe your privacy rights were violated, complain to us ([email protected]) or to the U.S. Department of Health and Human Services, Office for Civil Rights (hhs.gov/ocr). We will never retaliate against you for filing a complaint.
